IVUS、OCTをはじめとする血管内Imagingは、諸外国と比べて高いPenetration Rateを維持しており、冠動脈プラークの評価や適切なデバイス選択をするうえで、有用な診断ツールである。一方、Non-invasive 検査であるCTは、我が国の保有台数が世界と比べて高く、冠動脈疾患を疑う患者における有用な画像診断ツールであり、冠動脈の解剖学的評価と同時にプラーク評価も可能である。しかしながら、心臓CTで得られた定量的情報 (血管サイズなど) を十分、PCIに活かしきれていないのではないかと考える。本シンポジウムでは、心臓CTをPCIに最大限活かすべく、血管内イメージングと対比させることで、心臓CTの定量的・定性的評価の現状について議論を深め、将来に向けたCT-guided PCIの可能性について深堀りしたいと考える。

医師の働き方改革に伴い、カテ室でもタスクシフト/シェアが推進されている。それにより各職種への業務の拡大や移行、職種の垣根を超えて、カテ室内のスタッフで協力し合えることは何かを考える。さらに、タスクシフト/シェアが進んだ世界での各職種の新たな役割や、業務負担や責任の増大に対する改善策などを踏まえ、10年後のカテ室はどうあるべきか各職種の職能を活かした今後の展望についても語り合いたい。また、医師の少ない地域でもメディカルスタッフが協力することで、患者に質の高い高度な医療を提供できる可能性がある。今後、メディカルスタッフの活躍の場が増えることでのメリットや、乗り越えなけれがならない壁など様々な視点からタスクシフト/シェアを見つめ直し、私たちメディカルスタッフが進むべき道をディスカッションとともに語り検討する。

胸痛は冠動脈疾患の代表的な症状であるが、その定義に含まれる症状群は比較的あいまいであり、定量的な評価も困難である。本シンポジウムでは患者の自覚症状と冠動脈疾患の診断前確率、侵襲的治療の適応を含めた治療方針との関連性について議論を深めることを目的とする。
ERにおいて胸痛を適切にマネージメントすることは、急性冠症候群を見落とさず早期に診断することにつながる。各領域の専門医による講演により、循環器診断学の基礎的項目である胸痛の診断への理解を深め、最新知識の獲得を目指す。
COURAGE 試験,ISCHEMIA 試験 そしてREVIVED-BCIS2試験などのTrialの結果によりPCIを取り巻くEvidenceはPCIへの逆風となってきている。PCIのEvidenceのoverviewを通して課題及び利点を理解しするとともに、PCI手技自体、できるだけ侵襲の少ない手技であることが期待されると考える。日本は低侵襲PCIでは世界をリードしていると言っても過言ではなく、低侵襲PCIの意義と今後の期待について議論する。
スレンダーPCIにおいて、カテが細径であっても特殊テクニックを要することは多くないが、ちょっとした秘技が助けになることは少なくない。これらの秘技はスレンダーPCIのみならず、全てのcomplex PCIにおいて有効であるため、本セッションではSCJに受け継がれている様々な匠の技を特別に公開し多くの方にマスターしていただきたい。
遠位橈骨動脈アプローチはネットの影響もあり急速に世界に拡散している。一方で、国内の浸透率の伸びは高くなく、穿刺部位の変化に対する国内の反応は、橈骨動脈穿刺が登場した頃を想起させる面もある。どうやって橈骨動脈アプローチが日本で普及していったかを振り返り、遠位橈骨動脈アプローチの現状と今後の課題を考えたい。
Primary percutaneous coronary intervention (PCI) for patients with acute myocardial infarction saves a life. However, although numerous PCI-capable centers are widely available in Japan, the access to healthcare services exhibits inequalities among geographic areas. In this session, speakers will discuss where PCI-capable centers should be located and how we can perform primary PCI in a sustainable manner for the future.
Primary動脈硬化を伴わない冠動脈疾患が注目をされてきているが、まだまだ未知のことも多い。PCI術者自体が、それらの疾患の存在に気づかないで治療をし、患者にベストな治療が提供できていない可能性がある。SCAD,MINOCA、TGCVそしてIgG4関連疾患に関して、Expertの先生に最近の知見とその治療法、今後の更なる研究にもつなげたい。各々の先生に興味深い演題名を考えて頂きたい。
高安動脈炎は頻度は低いものの、若い女性が比較的多く発症し、病型によっては重篤な経過をとる。冠動脈を巻き込む病変では、通常のPCIのみでは対応できないことが多く、薬物療法を含む統合的な診療が重要である。冠動脈疾患を合併した高安病の症例に対して、現時点でどのように対処すべきかという知識を共有する。
i) 本邦で行っている循環器診療のデーター：JROADでわかることとJROADの問題点、ii) 地域で行っているレジストリーのいい点・悪い点、iii) また先行して全例登録されている「がん」についての知見を伺い、現在のJ-PCIで明らかとなる点と弱点をあぶり出し、今後の展望に活かしていく。
近未来の安定冠動脈疾患のカテーテル治療とその周辺ーAIやリスクモデルの導入でどう変わるか？
現在、循環器内科医を志す医師は減少傾向になる。その中でも虚血性心疾患を主たる対象としてPCIを施行するインターベンション施行医を志す者は少なくなっている。この流れを変えて本分野を目指す若手医師を増やすことはCVITの存亡にかかわる重要なテーマである。CVIT会員の骨格をなすインターベンション施行医をいかに増やすについて本音で語る企画としたい。
CVITと日本腫瘍循環器学会との合同企画は2022年の横浜での学術集会においても開催されている。日本腫瘍循環器学は今後の発展が確実に期待できる分野であり、CVITの会員の医師も知識を共有することは必須である。是非とも継続した開催をお願いいたします。
